The Institute for Hormonal Balance

The Institute for Hormonal Balance was founded by Dr. Gary Donovitz. Patients have entrusted Dr. Donovitz with their gynecological wellness and advanced hormone optimization since 1985 when he began his practice in Arlington, Texas. In 2013, the Institute welcomed Dr. Donovitz’s daughter Dr. Mandy Cotten, Family Nurse Practitioner, to carry on the practice. The Institute’s goals are to provide holistic health care to males and females alike, and is founded on the premise that  hormone optimization is the core of leading a healthy lifestyle. Each patient is treated as an unique individual and personalized treatment plans are created to ensure that the individual’s needs are met. The Institute provides custom hormone optimization plans that benefit patients’ quality of life as well as providing long term protection from disease processes. The Institute strongly believes in the importance of a partnership between provider and patient and supports patient input when developing and adjusting treatment plans. The Institute also provides services including well woman exams, gynecological problems visits, contraception counseling and initiation, nutraceutical management, SlimShot™  and other dietary guidance plans, IV nutrient therapy and  multiple top of the line aesthetic procedures.

MANDY DONOVITZ COTTEN, D.N.P., A.P.R.N.,F.N.P.-C FAMILY NURSE PRACTITIONER


I graduated from Texas Christian University with a Bachelors of Science in Psychology, and then returned to TCU a few years later to complete my Bachelors of Science of Nursing (BSN). Upon graduation I got my dream job working in Labor and Delivery at John Peter Smith hospital. Over the following 5 years at JPS, I worked in Labor and Delivery, Recovery, and was a team leader on an all women's unit. While working at JPS, I completed my Doctorate through Loyola University in New Orleans. I completed my clinical hours at JPS and Baylor in Fort Worth. I graduated with my Doctorate of Nursing Practice in May 2015, and am now a Family Nurse Practitioner certified through the American Association of Nurse Practitioners (AANP).

I have always been passionate about helping others and holistic health care. I feel that my position as a Family Nurse Practitioner at the Institute for Hormonal Balance I can fulfill this passion. I believe in providing education and options to my patients which I can do through annual visits and hormone balance. I have the privilege to follow in my father's footsteps (Dr. Gary Donovitz) and take care of the patients he has cared for during the last 30 years.

During the completion of my doctorate, I married the love of my life, Casey and we welcomed our first daughter, Rylie in the summer of 2014. I get to do what I love everyday, taking care of my patients and providing them with the best care possible and then I enjoy spending time with my family in my free time.

KIM INGRAM, RN


Kimberly Ingram, BSN, RNC-OB has 24 years of nursing experience. Twenty years in women’s health and four years in ICU. Kim has had one on one training with an expert Allergan practitioner earning certification in Botox, Voluma, Juvederm, and Kybella. Kim has a very gentle and calming personality and pays very close attention to detail. Kim’s goal is to develop a personalized plan for enhancing her client’s natural beauty with non-surgical injection technique.

KYLIE BUNNER, DNP, APRN, FNP-C


Kylie M. Bunner is a Family Nurse Practitioner who specializes in hormone optimization and disease prevention. She attended college at The University of Central Oklahoma, where she completed her Bachelor of Science degree with a major in Nursing Studies. Upon graduation she worked as a Registered Nurse in the Emergency Department.

She earned her Master of Science degree and successfully completed the Family Nurse Practitioner program at Texas Woman’s University in 2007. Kylie has been board certified by the American Academy of Nurse Practitioners as a Family Nurse Practitioner since 2008. Kylie is currently attending the University of Alabama in pursuit of her Doctor of Nursing Practice degree.

Kylie has been a certified BioTe ® provider since 2012 and currently serves as a BioTe ® proctor assisting other providers in learning the BioTe ® method for pellet insertion. Kylie lives in Prosper, TX with her husband Eric their three children Avery, Kirby,and Rigley. Her hobbies include exercise, all things sports and lake time with her family.

Kara Kramer, APRN, MSN, FNP-C

Kara Kramer is a Family Nurse Practitioner from Wichita, KS, who specializes in hormone optimization and disease prevention. She graduated from Newman University in 2007 with a Bachelors in the Science of Nursing. After graduating she worked in the clinic setting several years before returning to school.


Kara received her Masters of Science in Nursing degree from Wichita State University in 2014. She was board certified through American Academy of Nurse Practitioners in 2014 as a Family Nurse Practitioner. Kara has been a certified Biote provider since 2020.


Kara Lives in Haslet, Tx with her husband, Travis and 2 children Joey, and Khloe. She enjoys spending time with her family, traveling, and the lake.

Gary Donovitz, MD founded his private practice in 1985. From the beginning of his medical career, Dr. Gary Donovitz has sought to provide high-quality care to men and women from all walks of life.

A major benchmark came for Gary Donovitz, MD when he founded BioTE® Medical, the largest provider of natural hormone pellet therapy in the United States. As founder, chairman and medical director of BioTE®, Dr. Donovitz has spearheaded efforts to train physicians and healthcare professionals from across the country in the use of bio-identical hormone pellet therapy.

He has been a leading innovator in bio-identical hormone replacement for 20 years. He is a champion of changing healthcare through an individualized, comprehensive method of hormone optimization. He is an expert and international lecturer in sub-cutaneous hormone pellet therapy, having performed more than 50,000 pellet insertions and having successfully taught the technique to nearly 1,000 practitioners nationwide.

Additionally, Dr. Donovitz has been a pioneer in robotic surgery and has trained physicians across the country on how to perform operations using this procedure. He is a fellow in the American College of Obstetrics and Gynecology, a fellow in the Royal Society of Medicine, and the current medical director for the Institute for Hormonal Balance.
